Referral Information

A referral from a physician or other healthcare provider is not required for you to book an appointment with our physiotherapists, dietitian, or massage therapist.

However, if a member of your healthcare team recommended / referred you to see our allied health team, please provide us with their name and we will be pleased to communicate with them.

Booking Information

You can book an appointment with a member of our physiotherapy and allied health team online, or by calling 403 760 2897 ext 3

We are located on the second floor of the Shops of Canmore at #213 300 Old Canmore Road, Canmore Alberta.

The clinic can be accessed via an elevator or stairs and is accessible for all. Please enter through the door labelled second floor which is between Edo Japan and Freshii.

Diagnostic Imaging

If you are seeing a member of our physiotherapy team and have had diagnostic imaging originally prescribed by a practitioner external to Banff Sport Medicine, please bring the results with you.

Private Health Insurance Information

If you have private health insurance and would like to claim your treatment through your insurance provider, they may require a physician referral. Please contact your insurance provider so that you can confirm if you need a referral before your initial appointment.
